Medline Acquires Leading Long-Term Care Vitals Solution Provider that Integrates with EHR Partners

Medline and Constant Care Technology have announced an agreement for Medline to acquire Constant Care’s CareConnection, a software platform that saves clinicians time by automatically uploading vital signs into electronic health records (EHR).

CareConnection is the leading vitals solution in the long-term care industry that integrates with EHR partners like PointClickCare, MatrixCare, American Health Tech and more. The platform eliminates the need for manual entry of vitals and includes built-in predictive analytics that provide early indication of conditions such as sepsis and pneumonia.

Of the 14,959 skilled nursing facilities subject to the CMS’ Skilled Nursing Facility Value-based Purchasing Program, 73 percent received a penalty for hospital readmissions.  The new Patient-Driven Payment Model (PDPM) will make it easier for patients and providers to determine which nursing facilities excel at care for patients with certain conditions like heart failure.  Monitoring vital signs is a core function of nursing care that can provide early warning to healthcare providers of changes in patient condition. In a long-term care setting, like a nursing home, vitals are usually taken manually and reported multiple times a day for sometimes hundreds of residents. This can be a time drain time for nurses and certified nursing assistants (CNAs), and can often lead to errors, which CareConnection can minimize.

As part of the acquisition, the CareConnection team will join Medline’s Diagnostics division led by Ben Duck.
